2026 Heartland Homes Turkey Trot 5K Fun Run and Walk

RELEASE OF LIABILITY AND ASSUMPTION OF RISK

In consideration of being permitted to participate in the Heartland Homes/Options in Community Living, Inc. Turkey Trot 5K and related activities ("Event"), I acknowledge that running and participating in recreational events involves inherent risks, including but not limited to falls, contact with other participants, weather conditions, road and trail conditions, traffic, equipment failure, and other risks that may result in personal injury, illness, disability, or death.

I certify that I am physically able to participate in the Event and assume all risks associated with my participation.

To the fullest extent permitted by Minnesota law, I hereby release, waive, and discharge Heartland Homes/Options in Community Living, Inc., Essentia Health-Park Rapids Clinic, CHI St. Joseph's Health, the City of Park Rapids, Event sponsors, volunteers, organizers, officials, and their respective officers, directors, employees, agents, successors, and assigns from any claims, demands, damages, losses, or causes of action arising from or related to my participation in the Event, including claims arising from the ordinary negligence of the released parties.

I understand that this release does not apply to conduct that constitutes greater than ordinary negligence, reckless conduct, or intentional misconduct to the extent such claims cannot be waived under Minnesota law. Minnesota law permits waivers of ordinary negligence in recreational activities but does not permit waivers of greater-than-ordinary negligence.

I authorize Event organizers to secure emergency medical treatment for me if necessary and understand that I am responsible for any resulting medical costs.

I grant permission to Heartland Homes/Options in Community Living, Inc., Event sponsors, and their representatives to use my name, likeness, photograph, video, or other images taken during the Event for promotional, advertising, educational, fundraising, or other related purposes without compensation.

I understand that registration fees are non-refundable except as determined by the Event organizers in their sole discretion.

If any provision of this agreement is held invalid or unenforceable, the remaining provisions shall remain in full force and effect.

By signing below, I certify that I am registering myself and any additional participants listed on this registration form. I acknowledge that I have read and agree to the Event Waiver-RELEASE OF LIABILITY AND ASSUMPTION OF RISK. For any participant under the age of 18, I certify that I am the participant's parent or legal guardian and have authority to sign on their behalf. For adult participants, I certify that I have obtained their consent to register them and agree to the event terms on their behalf.
